Incidence of death and MI associated with stopping clopidogrel after ACS

Rates of death and myocardial infarction were assessed for a national sample of acute coronary syndrome patients after stopping clopidogrel. In the first 90 days after stopping treatment, patients experienced a nearly twofold increased risk of adverse events compared with subsequent follow-up intervals for patients treated medically without stents and for patients treated with coronary stents. This suggests a possible clopidogrel rebound effect, but additional studies are needed to support this hypothesis and to identify strategies to reduce early events after clopidogrel cessation.

Angiotensin-converting enzyme inhibitors in coronary artery disease and preserved left ventricular systolic function

We performed a systematic literature review and meta-analysis to evaluate the efficacy of angiotensin-converting enzyme inhibitors in patients with coronary artery disease and normal systolic left ventricular function. Angiotensin- converting enzyme inhibitor use was associated with modest benefits, which included a reduction in cardiovascular mortality, nonfatal myocardial infarction, and revascularization rates.

Pegylated Liposomal Doxorubicin (Doxil) and Drug Combinations in Multiple Myeloma

Current treatment approaches for mutiple myeloma focus on improving progression-free and overall survival. Although five-year survival rates have increased significantly over the past ten years, the prognosis of the disease is generally poor. The availability of several promising new therapies offers clinicians treatment options to further improve response rates, especially through the development of new combination regimens.

Early statin treatment after acute myocardial infarction

To evaluate whether the early use of statins would reduce cardiovascular events, we conducted a prospective, randomized trial that included 486 patients with acute myocardial infarction and normal total cholesterol levels, of whom 241 patients received statin therapy. The statin group had a lower risk of congestive heart failure and symptomatic myocardial ischemia, which indicates that early treatment with statins reduces recurrent cardiovascular events.

Are HDL cholesterol levels clinically significant in patients with low LDL cholesterol levels?

There is some debate about whether the risk of cardiovascular events associated with low high-density lipoprotein (HDL) cholesterol levels is attenuated by very low low-density lipoprotein (LDL) cholesterol levels (< 60 mg/dL), which can be obtained with statins and combination therapy.

Community cardiology clinics for secondary prevention of coronary heart disease and heart failure in primary care

Coronary heart disease (CHD) and chronic heart failure (CHF) are common chronic conditions encountered in primary care. Studies have shown that despite a strong evidence base, these conditions are often poorly diagnosed and inadequately managed in primary care.
