May 16th 2013The FDA has approved the tumor necrosis factor (TNF) blocker golimumab (Simponi) for treatment of moderate to severe ulcerative colitis in adults. No Evidence Backs Intra-articular Glucocorticoids for Acute Gout
May 16th 2013They may be supported by guidelines and commonly used in practice, but joint injections with glucocorticoids for acute gout are not backed by any randomized controlled clinical trials, according to a new Cochrane report. However, the report backs away from saying they should not be used.
Why Fibromyalgia Belongs in Primary Care
May 13th 2013New in the non-rheumatology journals, much about pain: A review based on 2012 fibromyalgia guidelines from Canada, a viewpoint on safe use of opioids, the evidence to back spinal pain interventions. Also: fast-track arthroplasty, and more newfound genes.
Pregnant Woman With Aching Pain, Muscle Fatigue, and Rash
May 2nd 2013A 39-year-old woman, in her eighth week of pregnancy, experienced aching pain that worsened with activity. Weakness and fatigue set in three days later in her thigh and proximal arm muscle. Then a mildly pruritic rash developed across the forehead, nose, and forearms.
Macrophages and CD4+ T Cells Found Lurking in Arthritic Knees
May 1st 2013Synovitis is present in 90% of painful knees, and emerging data is showing that this inflammation is "not just a bystander" in osteoarthritis-induced damage. Research is revealing the roles of macrophages and T-cells, and signposts toward new drug targets.
Teamwork Yields New JIA Genes and Plans to Explain RA Drug Response
April 29th 2013Two major examples of "crowdsourcing" genetic explanations for rheumatic diseases: a competition to explain the genetics of anti-TNF response in rheumatoid arthritis, and 14 newfound genes for juvenile idiopathic arthritis.
Some Patients Classify RA Without DMARDs As “Worse than Death”
April 26th 2013Pain, function and quality of life scores from 11% of patients with early rheumatoid arthritis rate the condition as “worse than death,” according to French researchers, with extreme pain and discomfort being a key determinant.
Exercise and Knee OA: Five Things We Know (And Some We Do Not)
April 25th 2013Patients with knee osteoarthritis should have mild exercise every day to reduce inflammation, should walk with shorter steps to reduce knee loading, and should strengthen their upper leg muscles, to judge from new research. But studies have yet to say how much exercise is best, how often, and at what intensity.
New Diabetes Management Guidelines Consider Concurrent Conditions
In diagnosing a patient with type 2 diabetes, an endocrinologist or primary care physician may struggle with developing healthy treatment targets when comorbid conditions like obesity and hypoglycemia are at play.
Small Nerve Fibers and Fibromyalgia: Five Signposts to a New Paradigm
April 23rd 2013A discovery that patients with fibromyalgia showed signs of small fiber impairment not present in depressed patients or controls may point to the first objectively measurable alteration of the peripheral nervous system in this disorder.
Chondroitin Sulfate Use Linked to Reduced Need for Knee Replacement
April 20th 2013OARSI 2013: More support for the supplement chondroitin sulfate (CS) in osteoarthritis: In a large observational study from Spain, six months' use of CS significantly reduced the likelihood of having total knee replacement. Observers noted the well-known issues in applying this knowledge in the US.
Lumbar MRIs and Baby Boomers: What Are We Looking For?
April 17th 2013The ACR's new list of probably unnecessary procedures set this emeritus rheumatologist wondering about another procedure that might be unwarranted: MRI for low back pain or sciatica in an older patient with no red flags. What does it accomplish beyond satisfying curiosity?
Movement Disorders and Chorea May Go Unrecognized in SLE, APS
April 17th 2013Some patients with systemic lupus erythematosus or antiphospholipid syndrome may initially present with movement disorders, especially chorea. Physicians may not associate these symptoms with either disorder, thereby delaying diagnosis and effective treatment.
